What if I select an agency that is not part of the Care Connection Community?

If you submit a “Self-Referral”, and choose an agency that is not part of the Care Connection Community, Care Connection Navigator will contact you and assist you with the referral. Or, if you are speaking with a Care Connecttion Service Navigator, he/she can conduct a warm transfer over the phone.

What kinds of resources are available in the Search Directory?

The Service Directory contains information on State and local community agencies that provide services to older adults and individuals with disabilities. Such as-health care and childcare, job training, education and recreation, caregiver support, services for older adults, disability and social service information.

How can I search for resources in the Search Directory?

Optional Search Parameters will allow you to narrow your search results and allow you to focus in on the organizations, products and services that more accurately meet your needs. These parameters include searching by service categories, state, zip code, distance from zip code, location, and/or areas served.

What are the eligibility requirements for the different programs/agencies?

Eligibility requirements for the different programs/agencies will vary from program to program. Care Connection Navigators will assess your individual needs and gather personal information to and recommend programs/agencies that you may be eligible for.

Can I get help over the phone or in person?

  • You can speak to a Care Connection Navigator over the phone Monday – Friday, 8:00am – 5:00pm by dialing 1-855-YES-ADRC (1-855-937-2372)
  • Send a message through the contact us form.

How can I make the text larger or smaller?

At the bottom left under the site menu you'll find a "Larger Text” or “Smaller Text” button on every page. The screen will refresh in the selected text size. You can change the text size at any time during your visit to the site.

Why are search results found only in the 13-county Gulf Coast region?

We only provide services to residents in the 13-county Gulf Coast region. If you live in outside the Gulf Coast region and are looking for services provided by an ADRC in your area please click here for a list of other ADRCs in Texas.

How can I view more details about an Agency I see in the Search Results?

Click on the Agency name and all the details about the selected service will be displayed.
