Core Functions
Our core functions and services can be described in four categories:
NAVIGATION
Care Connection© assists customers by screening for needs and directing customers to the appropriate services and support. Navigation provides immediate assistance on a short-term basis to customers, in-person and by telephone.
CARE COORDINATION
When needs are long-term and complex, we provide support to ensure and monitor service linkages to address customer needs. Care coordination may include referring customers to organizations that provide case management.
WRAPAROUND SUPPORT
The key characteristics of wraparound include mobilizing family and community supports to help develop and implement a family-centered plan of care based on customer’s needs rather than service availability.
EDUCATION AND TRAINING
Care Connection© provides education and training to professionals and consumers related to aging, developmental and physical disabilities. Education and training opportunities are available in-person and online.
